Every survey we see these days, seems to reach the same conclusion. And – judging by the number of articles we find scattered over the internet – the publisher are eager to spread the word NGAL is the sexiest thing on the kideny scene today…

In this German article- the MUST READ survey by Dr. Devarajan  Dr Michael Haase  Dr. Anja Haase-Fielitz, P.and Dr. Bellomo  is again the center of atention – undoubtely it helps that some of the other scientists involved are German 🙂  this quote is a fine example,,,

Die Forscher interessierten dabei zwei Fragen: Können die Biomarker dem Arzt helfen, eine akute Nierenschädigung frühzeitig zu erkennen? Und lässt sich damit der Schweregrad der Nierenerkrankung und damit das Risiko des Patienten, zu sterben oder eine Dialyse zu benötigen, abschätzen?

Auf beide Fragen lautet die Antwort Ja. Allerdings entpuppte sich nur einer der fünf getesteten Biomarker als guter Indikator, um eine akute Nierenschädigung zu erkennen oder auszuschließen. Das NGAL genannte Eiweiß wird in den Zellen der sogenannten Nierenkanälchen (s. Grafik) gebildet.

The Researchers asked themselves two questions: Can biomarkers help  doctors detect early acute kidney injury?  If so, can it measure the  severity of renal disease and assess the risk of the patient dying or of his/her need for dialysis ?

On both questions the answer is yes. However, only one of the five tested biomarkers proved to be a good indicator to detecting AKI or  excluding AKI.   The protein known as NGAL, it is formed in the cells of the  kidney tubules (see chart).
